| 内容記述 | Background: In 1996, disaster base hospitals began to be established in every prefecture of Japan. However, some issues concerning compliance with designated requirements still remain. Purpose: Prefectures differ in terms of the number of designations and designation requirements as well as disaster systems and response capabilities. Therefore, this study aimed to comparatively examine these differences. Method: A questionnaire survey was conducted in four prefectures with the maximum number of designations and four prefectures with the minimum number of designations. Results: The proportion of hospitals with emergency vehicles was 42% and of those with helicopter take-off and landing facilities was 62%. Further, 65% of the hospitals had implemented joint disaster drills with nearby hospitals, about 30% of hospitals had established arrangements with nearby hospitals, and a similar proportion had done so with nearby fire stations. The rate of compliance with designation requirements was higher for emergency medical centers and hospitals with more than 500 beds. Discussion: Although the rate of compliance with designation requirements was low and areas for improvement were identified, it was confirmed that emergency medical centers and large hospitals with more than 500 beds had established systems and procedures for responding to disasters. No significant differences were found between the group with many designations and the group with few designations, and the results indicated that the quality of disaster base hospitals does not change as a result of confirming their compliance with prefectural designation requirements. |

| 内容記述 | ［目的］ 1996年 (平成8年)、各都道府県において災害拠点病院の整備が始まったが、現在も指定要件の遵守に課題がある。都道府県によって災害拠点病院の指定数や指定要件が異なり、災害体制・対応能力の差があるため、指定要件の遵守率などを比較検討する。 ［方法］ 災害拠点病院の指定数の最も多い4県と、最も少ない4県にアンケート調査を行った。 ［結果］ 8県での平均の緊急車両の保有率は42%、ヘリコプター離着陸場の保有率は62%であった。近隣病院との合同災害訓練実施率は65％であり、近隣病院や近隣消防との取り決めがある病院は、30%程度であった。救命救急センターや、500床以上の病院になると、指定要件遵守率が高くなった。指定数の多い4県と少ない4県では、遵守率の差がなかった。 ［結論］ 指定要件遵守率は低く、改善すべき点がみられたが、救命救急センターや500床以上の大病院では災害への体制・対応とも整っていることが判明した。指定数の多い群と指定数の少ない群では有意差はなく、都道府県の指定要件遵守の確認行動により、災害拠点病院の質が変わるものでないことが分かった。 |
